WORLD AWARENESS SARCOIDOSIS DAY
Every year on April 13, people around the world wear purple for the warriors living with sarcoidosis. World Awareness Sarcoidosis Day calls attention to a rare and often misunderstood inflammatory disease, and to the strength of the community determined to outlast it. It is a day to learn the name, recognize the symptoms, and stand with the warriors.
Sarcoidosis is a rare inflammatory disease in which the immune system overreacts, forming tiny clusters of inflamed cells called granulomas in the body’s tissues. It most often affects the lungs and lymph nodes, but it can appear almost anywhere: the skin, eyes, heart, kidneys, and nervous system. Symptoms vary widely and may include a persistent cough, shortness of breath, chest pain, fatigue, weight loss, bone pain, and skin rashes or discolored patches. For many patients the disease is mild and manageable, sometimes with simple anti-inflammatory medicines, but for others it can cause lasting organ damage that reshapes daily life. Acute symptoms can include fever, swollen lymph nodes, and a rash known as erythema nodosum, while chronic disease most often troubles the lungs. Eye, heart, and nervous system involvement can be especially serious and require specialist care.
No one knows exactly what causes sarcoidosis, which is part of what makes it so difficult to diagnose. The prevailing theory is that an altered immune response to an unknown trigger sets it off, and certain genetic factors appear to raise the risk. The disease is also uneven in whom it affects: Black Americans, for example, experience a higher incidence of sarcoidosis, a disparity the Foundation for Sarcoidosis Research is working to address through its Ignore No More initiative, which pushes for broader clinical trial representation and better care for Black patients. An estimated 175,000 people live with sarcoidosis in the United States, and countless more around the world. Researchers continue to investigate which environmental exposures might trigger the disease in genetically susceptible people, and with fewer than 200,000 cases diagnosed annually in the United States, sarcoidosis is classified as a rare disease. The disease can affect the kidneys, brain, skin, and central nervous system in addition to the lungs.
Treatment depends on how the disease behaves. Many patients with mild symptoms need little more than monitoring, while more serious cases call for steroids, glucocorticoids, antimetabolites, or immunosuppressants, medicines that can bring their own difficult side effects. There is no known cure, which is why research funding matters so much. April was established as Sarcoidosis Awareness Month in 2008, and the movement gained a global anchor on April 13, 2019, when the Foundation for Sarcoidosis Research declared the first World Sarcoidosis Day. Since then, April 13 has united patients, caregivers, clinicians, and advocates in purple. The Foundation for Sarcoidosis Research, the leading nonprofit dedicated to finding a cure, improving patient care, and funding research, leads the global charge each April. Supporters are encouraged to post photos wearing purple and to tag @stopsarcoidosis with #StandUpForSarc during April.
Each year the community finds new ways to be seen. Landmarks light up purple, supporters post photos with hashtags like #WorldSarcDay and #StopSarcoidosis, companies join the Foundation for Sarcoidosis Research’s “Say Sarcoidosis” campaign, and fundraisers such as the Steps for Sarc virtual endurance challenge rally supporters to log millions of steps together while raising money for research and education programs. Past editions of Steps for Sarc have challenged supporters to collectively log nine million steps, the equivalent of 4,500 miles, while raising tens of thousands of dollars. Every conversation, every purple shirt, and every shared story brings research funding and earlier diagnoses closer. On April 13, we stand with every sarcoidosis warrior, seen and unseen, and we recommit to the search for better treatments and, one day, a cure.

Purple is the color of sarcoidosis awareness, and April 13 is the day the world wears it. Patients call themselves sarcoidosis warriors, and the purple ribbon is their banner. Landmarks glow purple, social feeds fill with #WorldSarcDay and #StopSarcoidosis, and the color becomes a signal of solidarity that patients can spot from across a crowded room. Spotting purple on a stranger on April 13 is an instant bond. If you wear purple on April 13, post a photo so patients scrolling their feeds can see they are surrounded by allies.
🩸 Granulomas: The Hidden Clusters
Sarcoidosis forms granulomas, tiny clumps of inflammatory cells, most often in the lungs. Because the disease can strike almost any organ, symptoms differ wildly from person to person, which is one reason diagnosis is so often delayed. Understanding the granuloma is the first step to understanding the disease, and explaining it helps others grasp why sarcoidosis is so hard to pin down. A patient with skin symptoms and a patient with heart symptoms may barely recognize each other’s illness.
🌸 From Awareness Month to World Day
April became Sarcoidosis Awareness Month in 2008, giving advocates a full season to educate the public. In 2019, the Foundation for Sarcoidosis Research declared April 13 the first World Sarcoidosis Day, giving the cause a single global moment each year when the whole community acts as one, in purple, across every time zone. The day gives researchers, clinicians, and families one shared date to rally around.
💖 Bernie Mac’s Legacy
Legendary comedian Bernie Mac lived with sarcoidosis and passed away from its complications in 2008. The Bernie Mac Foundation, created in his memory, funds research and raises awareness, reminding the world that this disease touches families everywhere, including the ones who make us laugh. His story opened countless doors for conversations about the disease. His foundation keeps his name attached to hope rather than only to loss. The foundation also supports comprehensive sarcoidosis care through the Bernie Mac STAR Center.
